Varicocele: Symptoms, Causes, Treatment, and Its Impact on Male Fertility

Varicocele is one of the most common yet often overlooked causes of male infertility. It affects approximately 15% of all men and up to 40% of men evaluated for infertility. Although many men experience no symptoms, varicocele can significantly impair sperm production by increasing testicular temperature and oxidative stress.

What Is a Varicocele?

A varicocele is an abnormal enlargement of the veins within the spermatic cord that drain blood from the testicles. It can be compared to varicose veins in the legs—the valves inside the veins fail to function properly, allowing blood to pool around the testicle.

Approximately 80–90% of varicoceles occur on the left side because the anatomy of the left testicular vein makes it more susceptible to increased venous pressure.

Why Is Varicocele a Problem?

The testicles are located outside the body because sperm production requires a temperature approximately 2–4°C lower than normal body temperature.

When a varicocele develops, blood flow becomes impaired, causing:

  • Increased testicular temperature
  • Reduced oxygen supply
  • Higher oxidative stress
  • Accumulation of metabolic waste products
  • Damage to the cells responsible for sperm production

Together, these factors can significantly reduce male fertility.

How Common Is Varicocele?

Varicocele is extremely common.

Studies show that it affects:

  • Around 15% of all adult men
  • 35–40% of men with primary infertility
  • Up to 80% of men with secondary infertility

However, not every man with a varicocele experiences fertility problems. The severity of the condition and individual susceptibility vary considerably.

How Does Varicocele Affect Sperm Quality?

Varicocele can negatively affect nearly every parameter evaluated during semen analysis.

Reduced Sperm Count

Many men with varicocele have a lower sperm concentration due to impaired sperm production.

Decreased Sperm Motility

Reduced motility is one of the most common findings. Because sperm require significant energy to swim effectively, oxidative stress caused by varicocele often reduces their ability to move progressively.

Poor Sperm Morphology

The percentage of normally shaped sperm may decrease, reducing the chances of successful fertilization.

Increased DNA Fragmentation

One of the most important effects of varicocele is increased sperm DNA fragmentation.

Excessive oxidative stress may damage sperm DNA, potentially affecting embryo development and increasing the risk of unsuccessful conception.

Lower Testosterone Levels

In some men, particularly those with larger varicoceles, testosterone production may also decline due to impaired function of Leydig cells.

Symptoms of Varicocele

Many men have no symptoms at all, and the condition is discovered only during infertility evaluation.

When symptoms occur, they may include:

  • A dull or aching pain in the scrotum
  • A feeling of heaviness, especially after standing for long periods
  • Discomfort during physical activity
  • Enlarged or twisted veins visible through the skin ("bag of worms" appearance)
  • A smaller testicle on the affected side

Symptoms often become more noticeable later in the day or after exercise.

How Is Varicocele Diagnosed?

Diagnosis usually begins with a physical examination performed while standing.

If a varicocele is suspected, the physician may ask the patient to perform the Valsalva maneuver (bearing down), which increases abdominal pressure and makes abnormal veins easier to detect.

The gold standard imaging test is:

Color Doppler ultrasound, which evaluates:

  • Diameter of the veins
  • Blood reflux
  • Testicular volume
  • Blood circulation

Varicocele Grading

Varicoceles are commonly classified into three clinical grades.

Grade I
Small varicocele detected only during the Valsalva maneuver.

Grade II
Varicocele can be felt during physical examination without the Valsalva maneuver but is not visible.

Grade III
Large varicocele that is visible through the scrotal skin.

Treatment Options

Not every varicocele requires treatment.

Treatment is generally recommended when:

  • Semen parameters are abnormal
  • The couple is experiencing infertility
  • The patient has persistent pain
  • Testicular growth is affected in adolescents

Treatment options include:

  • Microsurgical varicocelectomy (considered the gold standard)
  • Laparoscopic surgery
  • Percutaneous embolization performed by an interventional radiologist

Many studies have demonstrated improvements in sperm concentration, motility, morphology, and DNA integrity within 3–6 months after successful treatment.

Why Testicular Temperature Matters

One of the best-established consequences of varicocele is increased testicular temperature.

Numerous studies have shown that elevated scrotal temperature can impair spermatogenesis and reduce sperm quality. This is why maintaining the optimal temperature of the testicles has become an important area of research in male reproductive health.

Although cooling alone cannot treat the underlying venous abnormality, reducing excessive heat exposure may help minimize one of the factors known to negatively affect sperm production.
